We know health is more than just visiting a doctor or counselor. Our Care Coordination team helps address patient needs beyond medical and mental health. This allows the patient to be taken care of through a holistic approach, and removing the barriers that prevent them from reaching their health goals.
-
Our Care Coordination team is made up of dedicated Integrated Health Specialists. Outside of intake and recertification appointments, our Integrated Health Specialists are there at an as needed basis for referrals, prescription assistance, and SDOH needs. They work alongside the medical and counseling providers, to ensure the best level of care.
At St. Joseph’s Neighborhood Center, we believe that exceptional health care starts with respecting who you are. Our team is dedicated to fostering an inclusive environment that welcomes patients of all races, sexual orientations, gender identities (LGBTQIA+-affirming), cultural backgrounds, and abilities. We work proactively to eliminate barriers to care and ensure every individual feels seen, heard, and valued from the moment they walk through our doors.
