Care Coordination services are provided to our active clients, whether in Medical, Counseling, or both.

Care Coordination

  • Intake

    New patient intake process including Social Determinants of Health (SDOH) Screening.

  • Navigators

    Works with Navigators from community partners like Molina Healthcare, Coordinated Care Services Inc. (CCSI), and MVP Healthcare to help clients obtain health insurance.

  • Referrals

    Provides referrals to local community agencies for food, clothing, and other services

  • Application Support

    Provides support with applications for Housing, Social Security, Social Services, and Prescription Support.

  • Financial Support

    Provides support through rent, utilities, some car repairs, and food.
