"You saved my life. If it weren't for the care of your Center, I wouldn't be here."

That was recently shared with us by a patient who had their first physical with St. Joseph's Neighborhood Center (SJNC). This patient was in dire need of a medical exam and presented with many medical issues. Our volunteer provider sent them to the hospital, where they spent several days in the ICU. With the help of our Care Coordination team, they were able to obtain insurance, relieving some of the fear behind such a long hospital visit.

Once they were discharged, the patient called SJNC to update our team on their progress. They shared that they are feeling much better, are fully insured, and are on their way to continue their care outside of the Center. Tearfully, they extended immense gratitude to SJNC, the Care Coordination team, and our volunteer provider.
